How to submit complaints to the NYDFS:

You can file a complaint with the New York State Department of Financial Services (DFS) online, by mail, by phone, or by fax:

Online

You can file a complaint online at myportal.dfs.ny.gov. You can create an account if you don’t already have one. 

By mail

You can mail a complaint to the DFS Consumer Assistance Unit at 1 State Street, New York, NY 10004 or 1 Commerce Plaza, Albany, NY 12257.

By phone

You can call the Consumer Help Unit at (877) 226-5697 Monday through Friday from 9 AM to 5 PM. 

By fax

You can fax a completed Report Fraud Form to (212) 709-3555.

Law enforcement

Criminal subpoenas and official requests for criminal matters should be submitted via email to legal@sfox.com.
